Bulletins, flyers, brochures, letters, public service announcements, or other materials used to inform the public of the event or program must include the following statement:
If you need a reasonable accommodation because of a disability to fully participate in this program/event, please contact [insert contact name, host organization/dept.] at [phone number]. Please make the request as soon as possible to allow sufficient time to arrange the reasonable accommodation
If you provide a phone number also consider adding 1-800 - RELAY TX (735-2989). Which is a free service that provides voice call access for people who are deaf.
NOTE: A program may designate a date by which it would like to receive reasonable accommodation requests. However, a program may not refuse to provide a reasonable accommodation because the request was not made by the designated deadline. The program must try to provide the reasonable accommodation.
If an event includes a meal, the registration materials should include the following statement:

A syllabus for a course shall include the following statement:
The University of North Texas makes reasonable academic accommodation for students with disabilities. Students seeking reasonable accommodation must first register with the Office of Disability Accommodation (ODA) to verify their eligibility. If a disability is verified, the ODA will provide you with a reasonable accommodation letter to be delivered to faculty to begin a private discussion regarding your specific needs in a course. You may request reasonable accommodations at any time, however, ODA notices of reasonable accommodation should be provided as early as possible in the semester to avoid any delay in implementation. Note that students must obtain a new letter of reasonable accommodation for every semester and must meet with each faculty member prior to implementation in each class.
